Meet the Designer

Hi, I’m Gina. I’ve worked in healthcare for over 15 years, both in direct patient care and in administrative roles. I’ve always been passionate about workplace efficiency and finding ways to make things run more smoothly.

Over time, I noticed a common issue: many small to mid-sized healthcare offices were struggling to create professional-looking forms, often relying on DIY solutions without any design experience.

I started Good Health Forms to help practices like yours. I redesign paper medical forms to look clean, consistent, and easy to use, making life easier for both patients and staff.

FAQ

HOW DO I GET STARTED?

You can get started by completing the contact form, or send an email to info@goodhealthforms.com and we can setup a time to talk! Once I get an understanding of what exactly you need, I will send over a proposal with a projected timeline and budget for approval.

DO YOU DESIGN PAPER OR DIGITAL FORMS?

Both! I actually prioritize designing paper forms as this is still most often our clients’ preferred method of gathering patient information, but I also provide support for those wishing to implement fillable PDFs.

WHAT IS YOUR TYPICAL TURNAROUND TIME?

Most projects are completed within two weeks of the projected start date, but each project will vary depending on complexity and client response time. A projected start and end date must be agreed upon before the design phase begins.

DO YOU PROVIDE PROOF-READING? 

Yes and no. One of the benefits of using GOOD HEALTH FORMS is that I also have healthcare experience, which means I have a basic knowledge of medical terminology, anatomy and physiology, and pharmacology. It’s actually very common to find medication names, anatomical terms, and conditions/diagnoses to be misspelled or misnamed on client forms, and I make sure to correct those in the redesign. However, I am still human and typos can happen, so I also require that clients proofread all final drafts themselves (or ask a trusted third party) and then sign off on them before final files are distributed.
